You may reach the Medicaid Home Health Program during normal business hours, 8:30 to 5:00 pm. The Home Health Program Manager will be happy to answer any questions you may have on program policies and claim resolution.

Conditions and criteria for Home Health Services
• Physician‘s order is required
• Services must be provided by a licensed and certified Home Health Agency

To check Medicaid Eligibility call 1-800-809-3040 or use the Web Tool.

To determine the number of Home Health visit claims paid in the fiscal year July 1 – June 30 call 1-800-809-3040 or use the Web Tool.

To request to exceed the 50 visit limitation for Home Health Services, print the request form and submit with supporting medical documentation to:
