The Medical Homes Network Program (MHN)

The Medical Homes Network (MHN) health care delivery model is a primary care case management program.

An MHN is comprised of a Care Coordination Services Organization (CSO) and enrolled primary care physicians (PCPs). Only PCPs may contract within the MHN as all services are coordinated through the CSO and PCP, and claims for the services rendered are paid by the SCDHHS. An MHN, like an MCO, offers enhanced benefits beyond what is offered under traditional fee-for-service Medicaid.
