  Clinical Services 

Clinic services are outpatient services and are limited to procedures performed by a physician, physician assistant, nurse practitioner, nurse midwife, or specialized nurse practitioner. For Rural Health Clinic (RHC) services, the services are covered when furnished to patients at RHC's, skilled nursing facilities or the patient's residence. For Federal Qualified Health Center (FQHC) services, the services are covered when furnished to patients at FQHCs.

Current Service Limitations: Maximum twelve (12) visits per year for patients age 21 or older; unlimited for pregnant women and children under 21 years of age.
