Healthy Options Program (HOP) – The HOP alternative is a fee-for-service program designed to promote medical homes for children.

The HOP pays enhanced rates for sick care (office visits), in exchange for the physician providing a medical home. The physician must offer access to medical care twenty-four hours a day/seven days a week and provide preventive care. This can be done by a phone line. Children are not locked in to the practice and the physician does not authorize other services. They are encouraged to coordinate care. Since the children are not enrolled, their access to other services is not controlled.
