  Transportation Beneficiary Information

In case of emergency, dial 911.

General information

  • Transportation is available for doctor appointments, dialysis, x-rays, lab work, drug store or other medical appointments.
  • To ask for a ride call at least 3 days before your appointment.
  • Call Monday – Friday 8:00am to 5:00pm.
  • To cancel a ride call at least 24 hours in advance.
