CLTC Case Management Scope Revisions

The following revisions will be incorporated into the South Carolina Department of Health and Human Services (SCDHHS) Community Long Term Care (CLTC) Scope of Service for Case Management effective November 1, 2014.

CLTC is broadening the qualifications for case managers to include: (1) individuals with a bachelor's or master's degree in a health or human services field (2) Certified Case Managers. All individuals will be required to have two (2) or more years of experience in assessment and care planning with clients. (Section E.)

SCDHHS is clarifying its policy on case assignments for newly hired case managers. Case managers cannot service more than 35 cases during the first 30 days of the initial training cycle; and no more than 55 cases from day 31st through 60 days of the training cycle. Caseloads may be increased to 85 after 60 days of training are completed. (Section F.)

SCDHHS is clarifying the language regarding the case management back-up plan requiring case management providers to notify CLTC Area Administrator(s) and Lead Teams Case Manager(s) when the back-up plan is in place. (Section H.)
