HCBS C4 Individual Facilities/Settings Assessment Process

PROVIDER ALERT
The South Carolina Department of Health and Human Services (SCDHHS) will begin assessing Home and Community-Based Services (HCBS) settings in May of 2015. This process is implemented as part of the agency’s Statewide Transition Plan. SCDHHS must ensure compliance with the recent updates to the Centers for Medicare and Medicaid Services’ (CMS) HCBS regulations.
 
Providers of HCBS (waiver services) will receive a written letter containing specific instructions on how to complete the assessment for their individual facilities. This letter will be mailed in early May of 2015.
