New and Extended Healthy Connections Medicaid and Healthy Connections Prime Health Plan Offerings Available
Healthy Connections Medicaid Adds Contract with New Managed Care Organization
The South Carolina Department of Health and Human Services (SCDHHS) is adding an additional managed care organization (MCO) to serve Healthy Connections Medicaid members. Humana will be contracting with SCDHHS beginning July 1, 2021. Providers should expect to receive correspondence from Humana as they begin contracting with providers and continue to develop their provider network.
New and Extended Health Plan Offerings for Healthy Connections Prime Members
Launched in February 2015, Healthy Connections Prime is an enhanced option for South Carolina citizens who have both Medicare and Healthy Connections Medicaid, otherwise referred to as the ‘dual eligible’ population. Healthy Connections Prime is designed to integrate all services provided by Medicare, Medicare Part D and Medicaid into a single set of benefits fully managed by a Coordinated and Integrated Care Organization (CICO).
Healthy Connections Prime currently partners with three CICOs:
-
Absolute Total Care
-
Molina Dual Options
-
First Choice VIP Care Plus by Select Health
SCDHHS is pleased to announce a three-year extension with these CICOs through Dec. 31, 2023.
In addition to the extension for the existing CICOs, three new CICOs will partner with SCDHHS and the Centers for Medicare and Medicaid Services (CMS) beginning Jan. 1, 2022. The new CICOs that will serve Healthy Connections Prime members are:
-
Healthy Blue by Blue Choice of South Carolina
-
Humana
-
United Healthcare
Providers may recognize these entities as they serve other demographics and populations in South Carolina. Providers should expect to receive correspondence from Humana as they begin contracting with providers and continue to develop their provider network.
Healthy Connections Prime is a demonstration project jointly administered by CMS and SCDHHS. This aligns with South Carolina's overall efforts to support the Institute for Healthcare Improvement's (IHI) Triple Aim approach, which seeks to improve population health, improve patient care, and reduce per capita health care costs. Thank you for your continued support of the South Carolina Healthy Connections Medicaid and Healthy Connections Prime programs.
