There are three Coordinated and Integrated Care Organizations (CICO) approved by the Centers for Medicare and Medicaid (CMS) and SCDHHS to provide a Medicare-Medicaid Plan (MMP) for Healthy Connections Prime members. The purpose of this section is to share the following types of materials that were developed for CICOs:

Healthy Connections Prime Advocate (Ombudsman)
The Healthy Connections Prime Advocate is the ombudsman for people enrolled in Healthy Connections Prime. Their role is filled by the South Carolina Department on Aging, and they can assist you if you are having problems with your plan. The Healthy Connections Prime Advocate's services are free.

If you need to contact your plan, please use the contact information on the back of your Member ID card. If you have misplaced your card, please use the number shown below for your assigned plan. The hours of operation are 8:00 a.m. until 8:00 p.m., seven days a week. Alternative technologies will be used on weekends and holidays. Calls are free.
