To Change Your Plan Enrollment Once You Are a Member
It is your choice to stay enrolled in a Healthy Connections Prime Medicare-Medicaid Plan, switch to another plan, or leave Healthy Connections Prime altogether. To make a change, visit the South Carolina Healthy Connections Choices website or call 1-877-552-4642, Monday through Friday, from 8 a.m. to 6 p.m. TTY users should call 1-877-552-4670. This call is free.

For Questions About Medicare Coverage and Services
Healthy Connections Prime is a demonstration project jointly administered by the Centers for Medicare and Medicaid Services (CMS) and the South Carolina Department of Health and Human Services (SCDHHS).
Medicare is the federal health insurance program for people 65 years of age or older, some people under age 65 with disabilities, and people with end-stage renal disease (permanent kidney failure requiring dialysis or a kidney transplant).
