SCDHHS appreciates your interest in serving infants and toddlers with special needs. All new and existing BabyNet providers should ensure that the following documents are on file with BabyNet State Office. See the BabyNet Enrollment Requirements for more information. 

The following documents, along with all other required documentation, will need to be submitted online through the Provider Enrollment Portal for all individuals who provide services to BabyNet eligible children:

Beginning Jan. 1, 2022, all providers, including individuals and organizations, seeking to enroll in the BabyNet program must fully complete and submit the required documentation online through the Provider Enrollment Portal. This change is applicable to all enrolling providers, including service coordinators (early interventionists) and assistants, who have only submitted a portion of their required BabyNet enrollment documents.

Any providers who have started the enrollment process but have not submitted all required documents must do so no later than Dec. 31, 2021, or those documents will be securely discarded. After Dec. 31, 2021, enrollment documents sent directly to BabyNet staff members will not be accepted; documents must be submitted through the enrollment portal. Any documents that were submitted before the deadline are being worked and you will receive a notification once they have been completed.  

The provider enrollment portal will allow providers to securely upload their enrollment documentation. Please note the system will not allow partial submission of documents. Providers should have all documentation ready for submission before logging into the provider enrollment portal.
