Healthy Connections Medicaid team members and managed care plans will never ask for money, gift cards or something else of monetary value via text, email or a phone call. If someone contacts you asking for something of monetary value to get or keep Medicaid coverage, please report it by contacting the Medicaid fraud hotline at (888) 364-3224 Monday through Friday from 8:30 a.m. to 5 p.m. or by sending an email to FraudRes@scdhhs.gov.

UPDATE:

NOTICE: As of Jan. 1, 2026 , Healthy Connections Prime (Prime), South Carolina's Medicare-Medicaid plan, has ended. Members who were enrolled in Prime are encouraged to switch to a new dual special needs plan (D-SNP). Some members may be automatically moved to a new plan if their current plan offers a D-SNP.

Under federal guidance, in January 2026 the South Carolina Department of Health and Human Services made changes for members who receive both Medicaid and Medicare. Healthy Connections Medicaid members who qualify for Medicare may also enroll in a Healthy Connections Medicaid managed care plan. Some rules and exceptions apply.

Healthy Connections Medicaid works with Medicare to help combine coverage with Medicaid when possible. This is done by partnering with health plans to offer D-SNPs.

Healthy Connections Medicaid members with questions about Medicare coverage or enrollment should visit Medicare.gov or call 1-800 Medicare (1-800-633-4227). Members can also visit the SCDHHS website for more information about D-SNPs .
