Press Release: SCDHHS Launches Online Portal for Address Change Request

COLUMBIA, S.C. - The South Carolina Department of Health and Human Services (SCDHHS) has launched a new portal at apply.scdhhs.gov that enables members to update their address and contact information online. Through the portal, Healthy Connections Medicaid members can change their address and contact information quickly and at a time convenient for them.

It is important for Healthy Connections Medicaid members to make sure their contact information is accurate and up to date. If SCDHHS does not have accurate contact information, Healthy Connections Medicaid members may miss information on renewing and maintaining their health care coverage. This is especially important when SCDHHS resumes its required annual eligibility reviews, which have been paused during the current federal COVID-19 public health emergency. SCDHHS will resume annual eligibility reviews when the federal public health emergency ends. This date has not yet been announced.

To update a home or mailing address online, the member’s Medicaid identification number or Social Security number, date of birth, phone number and new address is needed. Healthy Connections Medicaid members can also update their address and contact information by calling (888) 549-0820 Monday through Friday from 8 a.m. to 6 p.m. or visiting their local eligibility office.
