SUBJECT: Updates to the Quarterly Seclusion and/or Restraint Reporting Form

This is to inform psychiatric residential treatment facility (PRTF) providers that the South Carolina Department of Health and Human Services (SCDHHS) will update the Quarterly Seclusion and/or Restraint Reporting Form. SCDHHS is adding a start time and end time for seclusions and/or restraints to the form. The provider manual will also be updated to include this additional requirement. The updated form must be used for all seclusion and restraint reports beginning March 1, 2018.

The updated form will be available in the Psychiatric Hospital Services Provider Manual beginning March 1, 2018.
