Webinar on South Carolina’s Rural Health Transformation Application
The South Carolina Department of Health and Human Services (SCDHHS) will host a webinar to review the state’s Rural Health Transformation Program (RHTP) application. The RHTP is a federal initiative designed to strengthen healthcare access, quality and outcomes in rural communities. It is led by the Centers for Medicare & Medicaid Services (CMS) and created under the Working Families Tax Cut Act, also known as the One Big Beautiful Bill Act. CMS announced guidelines for states’ RHTP applications through a Notice of Funding Opportunity Number (CMS-RHT-26-001) Sept. 15, 2025. SCDHHS submitted South Carolina’s RHTP application Nov. 5, 2025. CMS will announce RHTP funding award amounts for federal fiscal year 2026 by Dec. 31, 2025.
South Carolina developed its RHTP application and outcomes-driven initiatives by leveraging longstanding relationships with state agencies, healthcare providers, advocates, community leaders and local and state officials. SCDHHS received more than 350 proposals with ideas for inclusion in the state’s RHTP application. SCDHHS organized the various activities into five major initiatives. They are:
- Connections to Care: Utilizing technology to connect providers and patients
- Wellness Within Reach: Bringing care directly to communities
- Leveling up: Taking successful pilot projects statewide
- Shoring up to Sustainability: Investing in infrastructure and workforce
- Tech Catalyst Fund: Seed funding for innovative, startup health companies
During the webinar, SCDHHS will discuss how it will seek to increase access to quality healthcare services, particularly those that prevent and manage chronic diseases, in South Carolina’s rural communities through these initiatives.
