Healthy Connections Medicaid team members and managed care plans will never ask for money, gift cards or something else of monetary value via text, email or a phone call. If someone contacts you asking for something of monetary value to get or keep Medicaid coverage, please report it by contacting the Medicaid fraud hotline at (888) 364-3224 Monday through Friday from 8:30 a.m. to 5 p.m. or by sending an email to FraudRes@scdhhs.gov.

As part of the Bureau of Quality, the Special Populations Program supports SCDHHS's mission and strategic initiatives by providing oversight and coordination for children, youth, and young adults with complex needs. Members who are experiencing serious mental illness, those involved in the child welfare or juvenile justice systems, and others who may benefit from multi-agency collaboration and intensive case management are managed in this program.

The Special Populations team works closely with providers to ensure timely access to care and the effective delivery of services. This includes coordination and monitoring across physical health, behavioral health, special needs, and social support services, helping to stabilize or improve members' overall health and well-being.
