Final Public Notice for the Statewide Transition Plan

The South Carolina Department of Health and Human Services (SCDHHS) gives notice that the revised draft South Carolina Home and Community-Based Services (HCBS) Statewide Transition Plan, required per Centers for Medicare and Medicaid Services (CMS) HCBS Rule (42 CFR 441.301(c)(6)), is available for public review and comment. The revised South Carolina HCBS Statewide Transition Plan will be submitted to CMS on October 28, 2016. It will be effective upon CMS approval.

A list of the revisions made to the draft SC HCBS Statewide Transition Plan, along with a full copy of the Plan, can be found at https://msp.scdhhs.gov/hcbs/site-page/statewide-transition-plan, at each County Department of Health and Human Services Office, and at each Community Long Term Care (CLTC) Regional Office.

Copies of this notice are available at each County Department of Health and Human Services Office, each CLTC Regional Office, at www.scdhhs.gov, and at www.scdhhs.gov/hcbs for public review. Additional information regarding this revision is available upon request at the address cited below.
