Phase II (In-Progress)
During Phase II, SCDHHS will transition coverage of HCBS waiver services, nursing facility services, and hospice services into the managed care delivery model for the population included in Phase I. This shift strengthens the integration of long‑term services and supports with members’ medical, behavioral health, and pharmacy benefits, allowing MCOs to coordinate care more holistically and effectively.
As part of this phase, SCDHHS will also begin implementing the consolidation of the Community Choice, HIV/AIDS, and Mechanical Ventilator Dependent waivers into a single, comprehensive waiver program, Healthy Connections – Long Term Care (HC-LTC). Consolidating these programs will streamline access to HCBS, reduce administrative complexity, and ensure more equitable, consistent services for all members. Integrating this new consolidated waiver into the managed care delivery system will further support coordinated, whole‑person care and enhance the ability of MCOs to address members’ needs across all care settings.
Under the managed care delivery model, Healthy Connections Medicaid members will gain access to enhanced care coordination and additional value‑added services offered by MCOs. Depending on the plan, these additional benefits may include wellness and disease‑management programs, expanded home‑ and community‑based supports, caregiver resources, telehealth services, and other offerings designed to address social drivers of health and support community living.
