Healthy Connections Prime is an enhanced program that combines all of the benefits of Medicare and Healthy Connections Medicaid under a single Medicare-Medicaid Plan to make it easier for members to get needed health services.
Healthy Connections Prime is now available Statewide!
As of January 1, 2022, at least one Healthy Connections Prime plan is present in all 46 counties in South Carolina. Eligible individuals living anywhere in the state can choose to receive both Medicare and Medicaid benefits from a single participating Medicare-Medicaid health plan (MMP).
