The Medically Complex Children (MCC) waiver serves children who meet hospital level of care (LOC), and have chronic physical/health conditions expected to last longer than 12 months. The participants must also meet the State-defined medical eligibility criteria which evaluates the child's dependency on medications, medical supervision, hospitalizations, skilled nursing services, therapies and medical specialists.
The South Carolina Department of Health and Human Services (SCDHHS) received approval from the Centers for Medicare and Medicaid Services to add self-directed attendant care as a service to the waiver. Interested families who have a child who is covered by the MCC waiver can begin requesting the service on July 1, 2025, by contacting their Waiver Nurse Case Manager. Additional information for families and providers about this new service is available below under "Children's Attendant Care Resources."

Self-directed children's attendant care is a home-based service for MCC waiver participants up to age 21. It allows the option for eligible parents, relatives or guardians to be paid hourly to deliver the extraordinary care their child needs for day-to-day activities. This option is self-directed and would be used instead of an in-home care agency and personal care aide (PCA).
Interested families who have a child who is covered by the MCC waiver can begin requesting the service on July 1, 2025, by contacting their waiver nurse case manager. Below you will find useful resources to answer your questions about CAC, refer a child to the MCC waiver, or find your waiver case manager.
